This innovation is designed to support primary health care providers, particularly GPs, to care for their patients with diabetes in their local communities. Patients will have access to diabetes educators through referral via the QAS referral pathway, their GP or hospital diabetes services.
For the patients it will mean that patients with diabetes in the Darling Downs region are seen by the right person in the right place in the right timeframe.
