The Statewide Patient Flow Collaborative hosted the Patient Flow Showcase to explore models of care, strategies, interventions, processes and initiatives that have improved key parts of the patient journey. Some of the models have been developed or expanded under Care4Qld Strategy funding. The Patient Flow Showcase also introduced the SAFEST Patient Journey Home Framework. A total of 11 sessions were held and can be viewed individually below or you can watch our wrap-up video.
