Determine scope for the GEDI team and GEDI service
The scope for the GEDI team needs to be identified early to determine the clinical areas in which the team will be working. Your GEDI service may be implemented across all EDs in the health service or be limited to one.
- Will the CNC be working in one hospital ED or will they be overseeing several EDs?
- Will the GEDI staff be within one health service area or one hospital?
- Will positions be full or part time?
- What number of GEDI CNs will be required?
The GEDI CNC, CN and GEDI physician’s scope of practice needs to be determined. See example formula for determining staffing levels. Ensure that key stakeholders are involved in these discussions and in establishing key requirements for the roles.
Your team needs to determine the types of patients you wish to focus on. For example, it might be more appropriate to include people over the age of 65 at your facility.
Patients seen by GEDI nurses in original South East Queensland site ED might include:
- Patients 75 years, Aboriginal and Torres Strait Islander 55 years, or any resident living/ residing in an RACF
- Functionally active prior to admission
- At risk of increasing community care or residential care needs
Patients excluded by GEDI nurses in original South East Queensland site ED
- Patients on dialysis
- Patients awaiting aged care placement
- Patients requiring significant rehabilitation e.g. acute stroke
