Advance care planning and care at end of life

Sometimes unwell patients aren’t in the position to tell us easily what their care wishes are, or what conversations they’ve had in the past. Patients are therefore encouraged to complete their advance care planning documents when they are well, so that when they get sick, we can ensure their wishes can be respected.

The Queensland Government is encouraging patients and the community to discuss their health care wishes with loved ones and their doctor. You’ll be able to see if your patient has an ACP or Statement of Choices uploaded via the ACP tracker on The Viewer.

Our ACP Tracker provides 24/7 access to any submitted and approved ACP documents, so we’re better able to customise our care to each patient.
