While palliative care services are a well-established model of care across Australia, the same results are difficult to achieve within the Torres and Cape because of unique community needs, geography and healthcare settings.

The framework for a pop-up care model to be discussed in this presentation is inclusive of palliative care principals, supports the community to identify gaps and provides resources to assist communities to meet their needs. The principles of the pop-up model are that it functions in a coordinated approach only when it’s required (i.e., it “pops up”). The model is flexible, sustainable, owned and developed by the local community and tailored to meet community needs. Importantly, it also increases the knowledge and skills base in palliative care for communities and clinicians.

Torres and Cape envisage the model of care will provide clinical, cultural, social, and emotional support to facilitate the patient in returning home for end-of-life care and to “die on country”.
