The healthcare needs of adolescents and young adults are different to those of adults. Gaps in healthcare delivery and poor healthcare experience can arise when these needs are not properly understood and catered for.
One such gap, particularly for young patients with chronic conditions, is the delivery of the right care and support during the transition from paediatric to adult healthcare services.
While definitions vary, for the purposes of this meeting we considered the age range 10 – 24 years.
At its final meeting for 2020, the Queensland Clinical Senate, in partnership with the Queensland Child and Youth Clinical Network, endeavoured to provide a better understanding of the unique needs of this special cohort and explored opportunities to improve healthcare services, planning and integration for adolescents and young adults.
Clinicians, consumers and health system leaders from across Queensland gathered online to explore realistic solutions after hearing from experts in adolescent care about:
- The unique social and healthcare context for adolescents and young adults
- The importance of effective transition into adult services
- Service gaps and best practice
- Examples from services that are getting it right
